Who is eligible?
You are eligible if you are a Virginia resident 60 years or older and your income falls below 150% of the Federal Poverty Line:

How do I apply?
Applications are open and all applications must be submitted on or before September 30, 2026. Please complete ONE (1) APPLICATION PER PERSON. Duplicate applications will slow down the application review process.
Apply online or download the Healthy Together app and apply through the app on your smartphone. Click here for instructions on how to download and open the app.
If you were enrolled in the program last year and made an account on Healthy Together, please use your 2025 login credentials to log in and apply for the 2026 program.
